THE FLOOD OF FILLERS

Beverly Hills dermatologist Stuart H. Kaplan, counts some of the biggest names in music and film as FILLERhis clients. Recently he spoke with Beauty Undercover about the flood of fillers on the market and which work best.
Beauty Undercover:  Tell us, which fillers are most prevalent in the marketplace?
Dr.Kaplan:  There are so many fillers, and they each have their own advantages and disadvantages.  The three I use most often use:

  1. Radiesse:  It contains almost double the amount of product per syringe than other brands.  It also lasts longer than most fillers.
  2. Sculptra:  This is a volumizer.  While most fillers correct lines, Sculptra can fill the hollows of the cheeks, and can also be used to raise and strengthen cheekbones.
  3. Juvederm:  This is a hyaluronic acid product, and has a very smooth consistency.  It is versatile, and can be use to fill the folds around the mouth, and plump lips as well. 

Beauty Undercover: What do you see that is new coming on the market?
DK:  Newer products are trying to find niche benefits that separate themselves from the numerous other fillers on the market.  The newest products coming out now will have an anesthetic incorporated with the filler to decrease pain (For example, Juvederm XC with lidocaine is waiting FDA approval).  Other advances will add longevity without being permanent.  The problem with permanent solutions is that it is permanent whether you are 30 or 70.
BU: Fillers are fantastic, but sometimes women overdue it and get that moon face.  How do you avoid it?  Do you often have patients ask you to over fill?
DK:  Of course.  Many times, patients come in with photos of models that are much younger, or have fuller faces or lips.  I explain that every patient’s anatomy and aging is different, and I need to customize the amount of filler to their face, and not someone else’s face.   There is a fine line in cosmetic dermatology where you can either be a pushover and give into patient demands, or be too aggressive and push unnecessary procedures. I am neither, and my patients respect me for it. 
BU: You often say the hands need to match the face.  In other words women make their faces look young but the hands don’t match.  How do you match them up?
DK:  The face and hands age in similar ways.  Both areas get wrinkles, sun spots, pigmentation, and lose subcutaneous fat.  We can match the hands to the face with laser to diminish fine lines and pigmentation, and inject Radiesse to plump the tops of the hands.
